Corticosteroids in rheumatoid arthritis

Abstract

This report has presented evidence to support a disease-modifying role for corticosteroids in rheumatoid arthritis. It would appear that the efficacy of these agents in retarding the destructive course of rheumatoid disease has been substantially underestimated. Consideration for more liberal use of corticosteroids in the management of rheumatoid arthritis is warranted. Further study on new approaches to corticosteroid delivery is proposed.

      Osteoarthritis usually involves one joint; while, rheumatoid arthritis normally attacks multiple joints as well as some organs. Conventional treatment of OA and RA includes intra-articular administration of glucocorticoids (Weiss, 1989); it has been recommended to reduce complications of oral administration and has been proved to produce symptomatic relief in joint inflammation. However, the systemic application is limited, due to a high incidence of serious side effects, especially related to long-term treatment; therefore, sustained release via carrier formulations can be significant to avoid the adverse effects and prolong the retaining time of the drug in the articular cavity over a period of some weeks.
